1/10/2024 EnchantaVaughn Approved 1.Behavior: QMHP-C conducted an individual session with Josiah. Upon arrival, Josiah appeared to be in a positive mood evident by his facial expressions that consisted of a smile. QMHP-C informed Josiah that this session was going focus on his ISP goals. 2. Behavior: Josiah displayed normal and cooperative behaviors throughout the session without needing redirection from QMHP-C. 1.Intervention: QMHP-C began the session by inquiring about Josiah's emotional well-being. QMHP-C used active listening and positive feedback to encourage Josiah to express his feelings and clarify his actions to why he was so upset. QMHP-C assisted Josiah on brainstorming the issues he had had at home. QMHP-C reviewed Josiah’s behavior at home and if he had experienced any symptoms of anger and anxiety at home. QMHP-C assisted Josiah in expressing how he was acting and behaving at home when asked to do something or not getting his way. QMHP-C used active listening and supportive statements to encourage Josiah, as he shared his thoughts on how he was treating his family 2. Intervention: QMHP-C used positive feedback and compliments to support Josiah with his cooperation in the session. QMHP-C discussed with Josiah some of the triggers he feels make him upset and angry. QMHP-C used active listening and supportive statements to support Josiah while expressing himself about being asked to do something or not getting his way. QMHP-C used suggestions to assist Josiah with understanding why he needed to follow directions and that he will always have to follow someone’s directions in life. 1.Response: Josiah was receptive to QMHP-C's suggestions for today's session. Josiah was able to follow directions and listen to QMHP-C suggestions to explain why he was so upset. Josiah stated that his parents kept asking him to shower and clean his room and he didn’t want too. Josiah was able to answer questions and express why he should follow directions and lose his attitude, evidenced by his ‘I know what you were going to say, but I just over reacted’, comment. Josiah seemed appreciative of QMHP-C’s feedback, evidenced by his ‘I realize you care and I still don’t know why I get so angry’ and ‘it is really hard to listen and have to always follow directions’, comments. 2. Response: Josiah was able to participate and discuss his triggers, he expressed that he enjoyed hanging with QMHP-C and learning ways to not get so angry and mean, evidenced by ‘I think that when you make me explain how I am feeling, I am understanding that I can’t react in anger’, comment. Josiah was able to participate in therapeutic conversations with QMHP-C on not saying mean and negative things. QMHP-C discussed the session with Josiah’s father and asked him for some support to encourage Josiah to follow directions and do what he was being asked.
